Heart Coach –
What’s Included?

  • A Registered Nurse is on hand for your initial assessment and to set you up with everything to get started. Your nurse will continue to support you throughout the program.

  • Access to Bluetooth Blood Pressure and Blood Oxygen Monitors which connect with our patient App and clinician portal.

  • Educational Modules – we regularly send you a range of tailored educational resources and content that aligns to your needs.

  • Sessions with our multidisciplinary team of Psychologists, Exercise Physiologists and Dietitians to provide holistic physical and psychological support.
