What Is a Chronic Condition Management Plan?

A Chronic Condition Management Plan (CCMP) or a GP Chronic Condition Management Plan (GPCCMP) is a personalised plan created by you and your GP to manage long-term health conditions. Since July 1, 2025, a single CCMP replaced older plans like the Chronic Disease Management Plan (CDMP) and Team Care Arrangement (TCA).

Accessing Valion Health services using a CCMP:
  • You have a chronic or terminal condition lasting six months or more.
  • Your GP believes you’d benefit from a structured care plan.
  • You’re with a registered MyMedicare practice, or will access the plan through your usual GP.
Help with allied health support

You can get up to 5 Medicare-subsidised allied health appointments per year (or 10 sessions yearly for Aboriginal and Torres Strait Islander people). That could include physiotherapy, dietitian, podiatry, psychology, exercise physiology, occupational therapy, or group sessions for conditions like type 2 diabetes as long as it is consistent with the care plan.

With the new changes, your referral doesn’t need to name a specific allied health provider. For example, if your GP refers you to exercise physiology as part of your Chronic Condition Management Plan, you can choose which exercise physiologist you’d like to see, including from Valion Health.

Rebate amount: As of 2025, the Medicare rebate through a CCMP at Valion for seeing a Dietitian, Psychologist or Exercise Physiologist is $61.50. 

Getting started
  1. Book an appointment with your GP and let them know you would like a CCMP
  2. They’ll review your condition(s), set goals with you, and create the plan so you can access Valion Health services. The Valion team will help to advise you on what allied health referrals you’ll need.
  3. Your GP will provide you with the care plan
  4. You can attend your sessions using Medicare rebates.
  5. Return for reviews with your GP to check how things are going.
Important details
What if I don’t want to see a GP?

If you prefer to pay privately or access other types of support (like online counselling or coaching), you don’t need a referral or Chronic Condition Management Plan. But without one, you won’t be eligible for Medicare rebates. You can also access your private health insurance for rebates if eligible.
